ALL FORMS MUST BE COMPLETED IN BLACK INK AND SIGNED NO TYPED SIGNATURES OR INITIALS. The SECNAV form when mailed should be back to back – not 2 separate pages. On the SECNAV 5512 form page 2 – ensure you answer AND initial #29 and initial #30, then sign & date #31. These requirements are being mandated from our Security department who processes the SECNAV 5512. Right now the MWR Guest Card decal on the windshield is NOT required. NO Fax allowed. NO FEE for background screening. Pass good for only 6 MONTHS but is renewable. You must be a US Citizen.
Attached please find Two (2) forms which are an application form for the MWR Guest Card program. The Naval Station Security department has changed the process for access to the base and is requiring all MWR Guest card patrons to complete the attached form 5512/1. Please complete and return to me either via email or mail. I am the sponsor on the form and my phone number is 904-270-5228. Complete both forms completely. You do not have to fill in the information on employment or question 28. I will contact you when the background investigation is completed to schedule an appointment for a picture for ID and verify your information. There is no fee for the background screening. I will need to verify your your Driver’s License, current proof of Vehicle Registration and Vehicle Insurance. Once I have received the forms the process will take 10 days to two weeks. The pass is good for six (6) months and is renewable. Must be a US Citizen. The Guest Card application cannot be completed on line but you can print it and fill it in. If you have any problems with opening any of the documents – I can fax or mail the document to you. My mailing address is MWR Department, P O BOX 280048, Attn Sandra Barrett, Naval Station Mayport, Jacksonville, FL 32228-0048. YOU CANNOT FAX FORMS DUE TO PRIVACY ACT INFORMATION. If you send the documents via email – they must be in a Word or PDF file format. SECNAV FORM 5512 – Complete questions 1 through 24 on page 1. You must list two forms of ID in section 14, Social Security Number and Driver’s License Information. I am your sponsor, you need to initial, answer questions #29, 30 and sign, date #31 on the second page. You do not have to complete questions 26-28. If sending via email MUST BE in PDF file only. The MWR Guest Card does not enable you to bring guests. Each person in your vehicle must already have their own base access ID such as a guest card, military/retired/DoD ID. Parents and/or legal guardians (Guest Card holders) are allowed to bring their children under the age of 16 with them. Children 16 years or older would need an MWR Guest Card. This guest card is for patrons to visit our Category C facilities: Windy Harbor Golf/Bogey’s Restaurant, Mayport Bowling Center and Fast Lanes Grille, Beachside Bingo, CPO Club Restaurant and Lounge (All Hands events) , Ocean Breeze Conference and Catering Center and Beachside Community Center (All Hands Club). If patrons want to visit anything other than these facilities they cannot use the MWR Guest Card to access the base. If you have any questions please call me at 270-5380. Sandra Barrett
